How We Solved It

AQBEE built an integrated telehealth and remote patient monitoring platform on Salesforce Health Cloud. Video consultations were embedded directly into the patient portal via a Twilio-powered telehealth module, with automated scheduling, appointment reminders, and post-visit summaries. MuleSoft integrated RPM device data — blood pressure cuffs, glucose monitors, and pulse oximeters — into Health Cloud in real time, with Einstein AI alerting care teams when readings fell outside personalised thresholds. Automated care pathways in Flow Orchestrator triggered outreach workflows, escalated critical alerts to on-call providers, and scheduled follow-up visits based on clinical protocols. A Tableau dashboard gave population health managers visibility into remote monitoring adherence and intervention outcomes.
