AQBEE implemented Salesforce Health Cloud as the central patient engagement platform, integrating all three EHR systems via MuleSoft FHIR APIs for real-time clinical data exchange. We deployed Data Cloud to resolve patient identities across systems, creating a single unified profile for each of the network's 2.3 million patients. Einstein AI risk stratification models were built to predict readmission probability, triggering automated care plan assignments and outreach workflows through Marketing Cloud. An Experience Cloud patient portal provided self-service appointment scheduling, lab results, and secure messaging.
